Group Adult Foster Care

Group Adult Foster Care (Medicaid Assisted Living) is a comprehensive service for individuals (22 years or older) with disabilities or chronic medical conditions who must have daily assistance in order to remain safely in their homes.

Services

  • Personal care services on a five to seven day schedule
  • Homemaking
  • Home Delivered Meals
  • Meal preparation
  • Transportation
  • Grocery shopping
  • Adult day health program
  • Care monitoring by a Registered Nurse and a Case Manager

Eligibility

  • MassHealth (Medicaid) participant or eligible for MassHealth
  • Live in subsidized housing
  • Need daily assistance with tasks such as bathing, dressing, toileting, meal preparation, and ambulation
  • Physician's approval for program admission

Fees

  • Services are state-subsidized
