Prevent Type 2 (T2) Lifestyle Change Program
The Prevent T2 Lifestyle Change Program is a year-long program (that meets once a week for an hour) designed for people with pre-diabetes. It is also designed for people who are at high risk for Type 2 diabetes and want to lower their risk. The Prevent T2 Program helps people achieve moderate weight loss by eating well and becoming more active. The program works by helping individuals lose a modest amount of weight through increased activity and better food choices, as well as group and coach support. Stay Strong – A Fall Prevention Program
This program is a one-on-one in-home program for people who would have difficulty participating in a Fall Prevention Program outside of their home because of mobility and fear of falling. The program is aimed at improving strength and balance to reduce the risk of falls and increase the participant’s ability to safely leave the home. A trained instructor will conduct five home visits to: Teach and demonstration strength and balance exercises that can be done at home, individually, or with the help of a caregiver; Help participants choose an appropriate exercise class in the community, as well as arrange transportation if needed.
