Behavioral Health Outreach for Aging Populations

The Behavioral Health Outreach for Aging Populations (BHOAP) team helps address unmet behavioral health needs of older adults through in-home counseling and case management support. Through addressing the client’s underlying behavioral health condition, such as anxiety or depression, BHOAP social workers partner with clients to help reduce social isolation, improve medical care coordination, and increase access to mental health and wellness supports.
Free. Open to residents of Cambridge and Somerville ages 60 and up.

Stay Strong – A Fall Prevention Program

This program is a one-on-one in-home program for people who would  have difficulty participating in a Fall Prevention Program outside of their  home because of mobility and fear of falling.

The program is aimed at improving strength and balance to reduce the  risk of falls and increase the participant’s ability to safely leave the  home.

A trained instructor will conduct four home visits to:

  • Teach and demonstration strength and balance exercises that can be done at home, individually, or with the help of a caregiver
  • Help participants choose an  appropriate exercise class in the community, as well as arrange transportation  if needed.
