Home Care Referral

To refer a person 60+ to the Somerville-Cambridge Elder Services Home Care Program, please complete the Referral Form below.

Referrals are processed by our Intake Specialists within 1 business day. Once processed, the consumer or caregiver can expect an Assessment Specialist to follow-up within 3 business days to schedule an in-home assessment.

As the Referral Source, you will be notified if the person you referred does not enroll in our program or discontinues services in the future. Please feel free to contact the Aging Information Center at 617-628-2601 if you have questions about the process or status of a referral.
